This document provides a tool for assessing which strategies are in place in a primary care practice when it comes to improving COVID-19 care. It also provides a mechanism for prioritizing practice improvement activities.

La herramienta de evaluación le hace a los participantes del estudio 10 preguntas relacionadas con sus necesidades actuales no cubiertas con respeto a vivienda, alimentos, servicios públicos y más. También ofrece la oportunidad de solicitar recursos de asistencia y está disponible en inglés y español.

Created by the Hispanic Services Council in Tampa, Florida (a RADx-UP project community partner). It asks study participants 10 questions related to their current unmet needs related to housing, food, utilities, and more. It also provides an opportunity to request resources for assistance and is available in English and Spanish.
